Registration

New Student Registration

The registration form and related documents are the final step to enrollment. Once everything is submitted, transportation can be arranged and a start date can be set.

Required Documents:

Birth Certificate or Proof of Birth Custody Documents
TN Immunization Record with Recent Physical (less than one year)
Medication Administration Form (if school is to administer any medications, prescription or over-the-counter)
Emergency Care Plans (adrenal insufficiency, asthma, allergies, diabetes, seizures, or special diets)

Back-to-School Registration

Registration must be completed every summer before the student can return to school in the fall.

Updated Custody Documents
Updated TN Immunization Record with Physical (Kindergarten & 7th grade)
Emergency Care Plans (adrenal insufficiency, asthma, allergies, diabetes, seizures, or special diets)
