COVID-19 Student Assurance

Parents and guardians of TSD students (all campuses) are required to submit the COVID-19 Student Assurance form indicating that they have taken necessary precautions with their student to prevent the spread of COVID-19 at all TSD campuses. This is a requirement and must be submitted before arriving to campus (weekly for residential students at TSDK and daily for all others). One form per student must be submitted.

Please review each of these questions and provide your response in the first question "Would you answer yes to any of the following questions:" below:

Has your child been in close contact with a confirmed case of COVID-19 within the past 14 days?
Is your child currently experiencing a cough, shortness of breath, sore throat, or stomach symptoms?
Has your child had a fever in the last 24 hours?
Has your child had new loss of taste or smell?
Has your child had vomitting or diarrhea in the last 24 hours?

*Note: If you answer yes to the first set of questions or your temperature is 100.4 or above, notify the transportation coordinator for residential students or the principal for all other students. Your child's temperature is required to be below 100.4 degrees for at least 24 hours before returning to campus.
